REQUIRED DOCUMENTATION AT CHECK-IN

• The original plus two copies of the Sun Fun 3v3 Cup Official Team Roster/Liability Release, completed with all required signatures affixed.  To download a the roster form click here.
• Proof of age.  Each partiticipant must provide a proof of age such as a birth certificate, passport, drivers license, USYS state association player pass, US Club Soccer player pass, etc.
• Medical Release Form.  RECOMMENDED only, not mandatory.  The medical release form authorizes treatment of a minor child by a physician if the parent is not present.  We recommend that Coaches have medical lrelease forms for players under 18 if their parent or guardian will not be at the tournament.  To download the medical release form click here.
