Teams will check in 60 minutes prior to their first game at the headquarters of the complex where they are playing. Teams will not be allowed to play until they have checked in. The following documents are required:

1. Player Roster/Release Form

This report is restricted to the staff of teams accepted into the tournament. If you are authorized, you should click LOGIN in the upper right corner and enter your access codes to log in. If you have lost or forgotten your access codes you may enter your email address to have them sent to you. For additional help, you may click the envelope on the menu bar to contact the tournament staff.

2. Player Passes

Player Passes will be checked against the Player Roster/Release Form

3. A Copy of your Current State Roster

We will keep the copy. Add information on any guest players to this copy. Please have the original available in case of discrepancies.

4. Medical Release Forms

Medical Release Forms will be checked against the Player Roster/Release Form. There must be a Medical Release Form for each player listed.

5. Permission to Travel Form

This is required for all out of state teams. If your state has not already sent one to us, please have a copy ready. US Youth Soccer Region III teams only need a Notification of Travel Form. US Club Soccer teams do not need this document.
