Documentation Required
The following documentation will be required at check-in on Friday, October 19, 2012 at Tournament Headquarters located at the Double Tree Bentonville: 301 Southeast Walton Blvd, Bentonville AR 72712, 479-845-7770

Please Bring:
* Four (4) copies of official state team roster with the following details written on it.
* All player numbers must be written next to player name on official state roster
* Current laminated USYSA player pass for each player
* Official State Birth Certificate
* Medical Release Form- Notorized
* State Player Cards with photo and completely laminated
* Travel Permit for your state association
* Insurance Card
* Guest player forms signed by both team coaches as well as all information listed above.
* Guest Players name, player number and jersery number must be written on the bottom of the official state roster.
* State approved travel papers for out of state USYSA teams
Format
USYSA rules with three (3) games minimum.
U8/9 6v6, U10/11 8v8, U12 11v11 & 8v8, U13+ 11v11
Note U8/9 will enforce offsides subject to the refs discretion. All other divison follow normal FIFA rules.
