For everyone's convenience, check-in will take place one hour prior to your first game on Saturday.

All documents are required to be presented to the venue coordinator.  Please look for this person around the referee area.

Parent Release Form

Player Passes

Medical Release forms

Birth Certificates (if available)

Game rosters printed.  **NO WRITTEN ROSTERS WILL BE ACCEPTED**

You will provide your game roster and player passes to the refs for check in of each game.
