Required Documents at Check-in 

Please note - this is a US CLUB sanctioned event and open to all teams. As a US Club tournament, the online check in requirements are different from previous events.   

Proof of Age:  Proof of age must be uploaded for all participating players, including guest players.  Proof of age documents can be 2023-2024 player cards, birth certificate, passport, or for older players, driver’s license.

Tournament Waiver -   Each parent must sign the tournament waiver or electronically sign the waiver during online check in. 

Medical Release: Each player must have his or her appropriate association's medical release.
